Please copy and fill this out and send to [email protected] with the email titled: HEROIC ICONIC SURVEY

Also include in the email the NAME you back under on the Backerkit, without this we cannot verify your pledge.

If you backed for MULTIPLE characters, PLEASE send a SINGLE EMAIL SURVEY for EACH CHARACTER!

If these requirements are not followed, we cannot guarantee that your survey will get filed properly.
