Surveys will ask for your address, your preferences for your early incentives (bookmark or keychain) if you were an early backer, what name you'd like thanked in the books as a supporter (if you wish), and confirm any other add-ons and confirm shipping. 

If you haven't completed the survey, it will be marked as incomplete on my side. I will attempt to contact you to fill it out. You don't have to feel bad or anything.
