I've got great news! I finally received the proof copy of the physical book and it was good to go. For everyone who ordered a physical copy and has completed the survey (with the exception of a few whose payment is not going through),  I have now placed the order for your physical book with DTRPG. The lead time on the print-on-demand process is very difficult to predict. Normally, they ship within about 2 weeks, but the proof copy took nearly a month to ship to me, probably because of end-of-year crunch.
